Catchment area

We can register patients from the following areas:

• Brinklow
• Kents Hill
• Middleton
• Monkston
• Monkston Park
• Oakgrove
• Wavendon Gate
• Woolstone

We have a strict catchment area and are unable to take from other areas.

Broughton, Brooklands and Glebe farm are outside our area. Please do not request to register if you live in these areas as your registration will be rejected.

Registering with the surgery

To register with the surgery:

• collect a new patient registration form from reception during opening times
• Alternatively our practice is now offering online registration for patients.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
