GDPR Patient Notice

As a Practice we keep and maintain records about you and your treatment, both electronically and on paper.  This information takes the form of personal details (such as name, address, date of birth, details of contact with us and family history) and anything else relevant to providing the best possible care.  For the purposes of this we are known as the Data Controller.

We use your information for the provision of both direct and indirect patient care (in-house and via referral to secondary care).  Occasionally it may be necessary to disclose your information as required by law (ie to Public Health to help prevent the spread of infectious diseases), in the interest of public safety or if you consent to us doing so to a third party (ie insurance company).

This Practice also contributes to medical research data collections and national clinical audits.  The information provided for these data collections will not directly identify you although may include some personal data such as date of birth or coded medical condition.

We adhere to the principals of the General Data Protection Regulations, the NHS Codes of Confidentiality and any other guidance provided to us. All sub-contractors are required to sign a confidentiality agreement before commencing work in the Practice.

The GDPR gives you the right to access any information the Data Controller may hold about you.  If you wish to access the information we hold we ask that you make a written request detailing exactly what information you require.  This may take us some time to process for you, so please give us as much notice as possible.   If you request this information is given to a third party we will require your written consent to do so.

If you identify any errors in the information held about you, please let us know.  However, please be aware that we can only change anything that is factually incorrect.  You can contact the Data Protection Officer via the usual Practice address.

You have the right to object to your data being shared with outside agencies.  If you wish to know more about this, please contact a member of our Team who will be happy to discuss this with you.

Once leaving our Practice your records will be forwarded to Primary Care Support Agency (PCSE) who have the government contract for transportation and retention of medical records.  It is their responsibility to ensure your records are transferred to your new GP, or archived after a patient has emigrated or died.  Further information about the retention of medical records can be found at www.digital.nhs.uk

If you are unhappy with any of our data handling processes please let us know.  Complaints are dealt with by 1) The Practice Manager 2) The Senior Partner 3) PCIG Consulting Ltd.  If you are still unhappy with our response you can complain directly to the Information Commissioners Office via ico.org.uk and selecting “raise a concern” or telephone 01625 545745.
